medication has been lost, stolen, or damaged, reach out to your VA pharmacy as soon as possible. TO REPORT A LOST OR STOLEN PRESCRIPTION: • Call 1- (Pharmacy Customer Service) Have your VA
IDENTIFICATION and PRESCRIPTION NUMBER on hand, if possible. Your pharmacy team will evaluate the request and may need to discuss it with your provider to authorize a replacement. For
controlled substances, additional procedures may be necessary, or they may not be eligible for replacement. RECEIVING CONTROLLED SUBSTANCES Certain prescriptions, particularly controlled
substances, may come with more stringent rules regarding refills or mailing. Your healthcare provider or pharmacist will offer guidance on how to safely receive and store these medications.
SAFELY DISPOSE OF YOUR MEDICINE Drop off your expired or unwanted medicine at a MedSafe box. Disposing of your medicines safely can help protect other people as well as the environment.
