for your Clinical Training. This web page has the information to allow you to receive your VA badge and computer codes by the start of your rotation. You must start this process a minimum of

eight weeks prior to your rotation date. FORMS TO COMPLETE: All forms must be COMPLETED IN THEIR ENTIRETY a minimum of eight weeks before the start of your rotation of your training

your rotation is complete. Required EIGHT WEEKS PRIOR to start of rotation, print this letter and fill in the specialty service, sign, date, print name, home address, and school address. Do
