Our Euclid Avenue multiuse clinic houses an outpatient VA Dialysis Center, and the Community Resource and Referral Center (CRRC) with a Homeless Patient Aligned Care Team (HPACT). HPACT is a
multi-disciplinary team designed around the unique needs of homeless Veterans and includes a nurse practitioner, a social worker, a physician, and a registered nurse, providing
comprehensive and individualized care to homeless Veterans to meet medical, mental health, housing, and financial needs. CRRC is open 8am - 4:30pm, Mon-Fri. Call for an appointment or with
